Living with dementia can be a scary and isolating time, not only for the individual but for family members too. That's why we are here to help. We can contribute positive renablement which means we focus on the things people can do instead of what they can't.
Individuals can live well with dementia with the right support and understanding. We provide meaningful stimulant activity to the person's ability which helps mentally and physically.
We can support to look after your loved ones at the end of their lives. We will understand how they want to be treated by personalising their care plan and adhere to any wishes they have. We will work with other professionals such as the palliative care team, GP's and District Nurses to understand different stages and how we can best support. All staff are compassionate and will show empathy at this stage of life. Allowing someone to pass at home can be the biggest gift and achievement and will endeavour to do this for you.
Personal care can stem from having a shower/bath or strip wash. Completing oral care and getting dressed. There are many different elements but the outcome is about helping you achieve your daily appearance and reducing any risks that may be identified.
A personalised care plan will completed and we will work with you to ensure you are getting the wants and wishes you hope for. Above all we are here to promote Independence .
Not everyone requires physical support, some just need someone to talk too or support them going out for appointments etc. We can take you on days out, shopping or just sit and have a cup of tea- whatever is required a care plan will be designed around your wishes. Don't feel lonely is a busy world!
If you are a carer or someone who looks after a loved one and require a break we can sit with your loved one or take them out into the community for a period of time to offer respite for you.
We understand how people may not want to go onto a care home so we can offer someone to come and live with you to enable you to remain at home. Your home must have a spare bedroom and depending on needs we can support with sourcing equipment etc for you.
